Patient Centred InteractionsManaging day-to-day challenges with chronic conditions! Better Choices, Better Health® is an interactive program where participants learn and practice using techniques and tools to help manage their day-to-day challenges living with chronic conditions. Some of these tools are setting and achieving goals, having productive interactions with healthcare providers, communicating better with friends and family, being more active, using problem-solving skills and managing symptoms. The Better Choices, Better Health® program supports people to transform knowledge into action so they can better manage their health and have a good quality of life.
Participants can self-refer or be referred to the program by a healthcare provider. It is offered in-person in communities across Alberta, as well as online at no cost.
Better Choices, Better Health® is an internationally recognized evidence-based self-management program developed by the Stanford Patient Education Research Center.
In this workshop you will:
- Learn to manage your symptoms better
- Discover tips to manage your day-to-day activities
- Get support from others with long-term health problems
- Share your experiences and help others
